In order to see if you may qualify for this study, we will ask you to provide us with some preliminary information. If you agree to participate in our online pre-screen, you will be asked to provide your contact information and information about your overall health and potential gynecological symptoms. The contact information you provide will be the method of contact used by researchers to reach you if you qualify for the study.
Your participation in this survey is completely voluntary. You can withdraw at any time by just exiting the survey. If you exit the survey and do not complete it, then your information/data will not be saved and the research team will not contact you.
Your participation in this online questionnaire does not involve any risk to you beyond that of everyday life. You are not likely to have any direct benefit from participating in the pre-screen. All of the information you choose to disclose on the questionnaire is confidential.
You may choose to remove your information at any time by contacting the research staff. You will not be paid for filling out the pre-screen. If you are eligible and participate in our study, compensation may be provided.
This survey is being hosted by REDCap and involves a secure connection. Study records that can identify you will be kept confidential by storing data on a secure server and allowing only trained research staff to review data. We only collect information that you choose to provide.
